Hair Collection Only
This service is for hair collection only. You must have paperwork and collection supplies to accompany the sample to the appropriate lab. If you need additional labs performed, add them in the add-on section on this page.
Therapeutic Phlebotomy
Therapeutic Phlebotomy is a blood collection for those with hemochromatosis and other underlying blood disorders. The blood draw consists of removing 450 to 500 milliliters or 1 pint of blood from an individuals arm.
Buccal/Nasal Swab
This service is for either a buccal or nasal swab collection. You must have paperwork and collection supplies to accompany the sample to the appropriate lab. If you need additional labs performed, add them in the add-on section on this page.
Urine Collection Only
Fast and hassle-free urine sample collection for lab testing. Book your appointment, provide your sample, and get accurate results—quick and easy!
Phlebotomy Blood Draw
This service is for individuals that have a physicians order and using insurance to cover the lab test. If insurance information is not on the lab requisition or physician order, please upload front and back of insurance cards in insurance section of this page or have it handy to show the technician upon arrival. Results will go directly to the ordering physician. If you have test that need to go to more than one lab, please add them in the add-on section of this page.
Specialty Lab Kit Collection
This service is for individuals that have been sent a kit from a specialty lab. If you have additional kits or test that need to be drawn and sent to different labs, please add them in the add-on section of this page.
Self-Pay Lab Testing
This service is for individuals that have already paid for lab test and only need a blood draw or specimen collected and delivered or shipped to appropriate lab. If you have more than one lab order, please add them in the add-on section of this page. You must have the lab order printed out already.
